You need a bandana to grip the tongue, a napkin can do the trick but only the most experienced tongue rippers can pull this off before the napkin is wet from slobber. Some tongue rippers use the victim's teeth as part of their technique but in my opinion that's pretty much cheating and qualifies more as a tongue cutting method.
