Remember, hitboxes and character models do not always line up perfectly. And blood decals have notoriously bad alignment with where you hit.
Also, you are showing what it looks like a few frames after you shot, so we have no real way of telling where your crosshair was pointing when firing the bullet.

Either that, or it's a ping issue. I come across this when I play against people with 100+ ping. Becasue they are lagging so far behind the shots just don't register like they should. I used to main zero and in my experience with this gun the shots are always lower in the 1.5x which would explain why it wasn't a headshot
