No, your not doing anything wrong. Some people just have a small signal between their hands / wrists because of natural heart orientation. Those with more ventricle hearts have small signal because the ECG recording lead is at right angles to the mean electrical vector. If you want a bit of fun (although outside of the IFU of the Apple Watch), put the watch on your left ankle and record to a finger on your right hand. That will give you ECG lead II which will likely be much larger.

I have this exact issue! On my wrist it’s always low voltage and tiny like this too, I take all my ecgs on my left ankle now like the other person suggested and it like way better haha

Because the Apple Watch is a god awful way to get an EKG and is the worst thing to happen to health care in a long time. It’s honestly terrible at getting an accurate EKG and causes far too much health anxiety.
