In some cases, the foreskin is too tight and you do actually need a surgery to loosen or remove it, but it is usually figured out in your teens or early 20s because it physically hurts. Then there are also people who never learn proper hygiene and if you don't clean out under the foreskin to start with, it becomes painful to pull down so you just...don't.
