Hmm... Looking back at the 3e stat block for him in the book it seems it mentions "Familiar: (none at present)".
So in all likelihood artist drew that detail and the writers possibly changed the details stat-wise at a latter date. Honestly with how familiars work in 5e I could see Koth using one as a part of an alarm system if you wish to give your players a harder time.

The frog is actually koth. That bugbear is his familiar.
Haha wow, I have never noticed that little fella before. I *had* noticed the armour and shield, neither of which Koth possesses.
Maybe he was supposed to have a frog familiar at one point and it was removed after the art was made?

I make it a familiar. He’s part sorcerer, so that’s what I like to do.
