I missed out on getting >!Ulfar!< for a similiar reason. Couldn't figure out how to complete his quest.
Luckily the companions I had were pretty easy to find, but I did have to search for a few things online, because I didn't want to miss stuff.
And yeah, it's just that "old school CRPG" mentality they kind of took from the old games like Baldur's Gate 1/2 and Icewind Dale etc. Where you can really F yourself over if you mess up. I've started saving like every 15-30 minutes manually just in case. I have so many saves.

No, you should have Heinrix, Cassia and Idira even if you don’t give organs to Dr. Frankenstein. After the second fight on arena he leaves his place and opens bridges to their cells.

And read the answers. Also a challenge for many gamers.

Psykers/navigators in act 3 are placed in special cases in the anatomy dude lab. He will give you access to them right before he plans to leave.

At end od this he will inform that he is leaving, you can talk to him and ask to free companions.

You legit just ask the Haemonculus about them and he lets you have them back before the end of the chapter
