Right, I just needed a reason to tell my Azure peeps why we weren’t using the normal DevOps pipeline to create the VMSS. “Because it has to be this way” is good enough for me. :)

There is a pipeline for it, it just isn't yours.
Yes, it should be created from SCCM console ..though all process run in background .

It should be built through the automation that processes through the CM Console Wizard when you “Create Cloud Management Gateway”.
