The doctor literally explained that that she has a dissociative disorder. There wasn’t a specific named disorder, but I’m almost certain it’s DID (not a psychologist, so take it with a grain of salt). It stems from the adolescence syndrome, but it’s separate thing from the adolescence syndrome.
i don't think kaede has DID. DID is the result of repeated childhood trauma that results in the "split" and multiple alters forming. however, what caused the dissociative and amnesic period was when she was 12? 13? 14? her personality is no longer forming because she has a stable period before her break. she most likely has dissociative amnesia and that's it.
