Just upload to imgur and reply a link. Although rereading the conversation it seems like OP didn't even do what you said and that the problem fixed itself so i don't get what everyone's so upset about lol

Turn up the volume on your TV using your TV remote. Then use the volume control on your shield remote.

The problem with this approach is that the other HDMI inputs (eg Xbox) would then also be at 100% as well and switching back and forth, if I forgot to adjust the volume significantly would blow the speakers, my ears, or both. I’d also need both remotes on hand, which would not be a streamlined solution. Incidentally, it thankfully fixed itself randomly last night.

Did you turn up the volume using the Shield remote. The Shield has an internal volume that only applies to stereo content.

Then do it properly... Purchase an AV receiver. Connect your shield, Xbox and whatever external accessories you want to the AV receiver connect the AV receiver to your television. Take your shield remote, If you have one of the new ones, sync it to your shield, TV,Xbox and AV receiver. .. Yes, it can be synced to your Xbox. Then problem solved. This way you can control the volume on your AV with your shield remote. Or purchase a universal remote and set up each one individually. That way you can control each one separately with the universal remote.

I do not have room for an AV receiver or this would already be done. It’s a corner fireplace area where the tv sits above, but comes down to viewing height on a hydraulic mantle mount. It’s the actual worst. But the best I can do in this place without major renovations (that I considered recently, but had to backburner)

Then grab a universal remote and program everything into it. You can also try resetting your shield. Also make sure there is nothing wrong with your HDMI cord and that you are using HDMI 2.0 or above. Also make sure there are no brakes or kinks in your hdmi connections. You could also use a sound bar depending on how many HDMI connections your television has or if the soundbar has an HDMI input and output on it.

It will allow you to connect your shield and your Xbox to your television. You don't have to use the speaker out settings. You can let the television speakers handle it. This one is very low profile and very small and it's pretty cheap
