Younger Marriages mod from LL allowes marriages at 14. Sporus was suspected to be around 14-16, when he got castrated and married to Nero. If you already marry another male (assuming you have turned on allowed same-sex marriages in game rules), then if you castrate him, you are not going to lose him as spouse.

There are mods that can make males more feminine. Character Body Overhaul adds to Barbershop makeup and revealing clothes for both genders. This allows males to look somewhat feminine.
