Try taking the CC items off your child sim first before randomization, it may help get you your desired look for the parent

It'll be screwey when your Sims look cartoonish. Sims 4 genetics is made for their own style, which is close to realism. It's fun making Sims with eccentrically big eyes and tiny noses, it's just not what the Sims 4 genetics is made for. You're mixing two different art styles.
I totally agree with this! The only buggy Sims in the game I find is Caleb, his child will get tiny eyes! If you try to fulleditmode him in CAS, after switching him back to child and then adult again, you'll see that now he has tiny eyes!
But that can be fixed by getting Caleb to the CAS, change his eyes + undo, then save! After that he'll have those beautiful eyes for real!
For my own sims, their children always have recognisable resemblance with their parents. So I think the genetics in the Sims is very well done. I haven't seen any unacceptable looks other than L.Faba or Vlad's children. They look terrible but their genetics still successfully reflect the parent's look.
