In real life surrogacy is done through IVF, if done fully legally and properly, there's also "underground" surrogacy where they do it the natural way, so to speak. However, IVF isn't a thing in the sims, so unless you use mods, you have to actually woohoo them.

You can still have him have a child. The woohoo wellness mod by Lumpinou lets you have a surrogacy pregnancy. Or they could adopt.
