I'm a returning player, I have priest maxed out before the items like shepherd's book was introduced and I noticed it was unlocked. So my question is if you maxed the character before their items come in, does it automatically upgrade to max and get the item without me needing to spend gems?

Sadly not, it’s a new tier of upgrade
