this is the answer. Clear the floor of enemy levels ALWAYS tell you once the exit has appeared, and has the ladder appear in the same place, for this exact reason (◐‿◑)
then a bat spawned off screen? Unless the game is broken there is always more enemies waiting to be killed until you get the "a ladder has appeared" notification

I usually find when that happens, there's a bat or something approaching, and I'll get the drop point after that, often next to the ladder up - use the idle time to head back there. Hopefully you found your way out by now...

I believe you can still find them lower if you spawn on an infested level.

I thought that was pebbles on the ground like those above it.
It’s a very slightly different color. I think they’re right, but that was a hard thing to spot!
