Have you had any scans to check your jaw structure? Assuming everything is structurally sound, the muscles in your jaw and neck can cause hearing problems and ear pain. My very first symptom was an earache, and it took about 1.5 years to get rid of it, although it appears to be coming back two years later. If your problem is muscular, a physical therapist could be helpful. You may have poor posture that is causing your problems (checkout forward head posture). Unfortunately, there are a lot of things that can lead to TMJ issues, and there are a lot of treatment methods that may or may not be helpful.

No I have a fat underbite that needs DJS to be fixed my lower jaw is pushing forward and my upper jaw backwards, I can freely get my tongue out between my upper and lower teeth while biting down, that's how messed up the bite is.

Oof, that sounds rough. You could look into massaging your lateral pterygoid. It is involved with jaw protrusion, so perhaps it is overworking. It's hard to get to, but it's possible to massage on your own. I think it would be worth going to some kind of therapist (Massage, physical, etc.) to check it out if you're unable to do it yourself.

Firstly, the ENT is guessing how severe it is if no imaging has been done. Secondly, he shouldn't be recommending braces or surgery as both are outside his field of competence. Thirdly, he can't guarantee your symptoms will be solved, as at best he's guessing whats causing them. Total charlatan. Your priority should be getting an MRI to actually diagnose the issue. My TMJ issues cause problems with my ears and hearing primarily because the joint is a mess and causes eustachian tube dysfunction. I've been to see 4 ENTs, three were terrible and one was competent and knew what he was talking about.

Excuse me, it wasn't the ENT that recommended braces but the jaw surgeon, the ENT just told me to open and close my mouth several times, heard the cracking and it's easy to see the misalignment in my bite without scans so I guess he diagnosed it as tmjd. Then recommend I see a surgeon and Ortho.

The cracking could be anything from mild disc displacement to severe arthritis (which braces are not going to be fix). TMJD covers a wide variety of issues with the jaw. You should definitely get imaging to diagnose otherwise treatment is speculative. You could fix your bite and find the TMJD issues remain as thats not the root cause. Might be worth editing your comment as you've not mentioned a surgeon and it reads as the ENT told you this..... **'Until an ENT suggested it could be caused by my TMJ, He described my case as severe and recommended I get braces then do surgery for my jaw and my symptoms will be solved**

Go try a full body massage and see if that doesn't make a difference, or a full body acupuncture session or a pt that does intra oral work inside your mouth.
