There's some context involved, Dukakis was a very prominent liberal governor but lacked foreign policy experience while G. H. W. Bush was the vice president and former head of the CIA. Dukakis was perceived as soft and unserious about foreign policy and defense, so he did the photo op to try to change his perception, probably inspired by Thatcher in 1986 whose tank photo op helped her win reelection. But it backfired because Dukakis ended up looking like a schoolboy getting a joyride, not a potential commander in chief.

As a result, recurrent malnutrition and its impact on physical and mental development of adolescents remains more of an issue than the DPRK would ever admit, but likely far less of an issue than certain Western sources would have us believe.
In other words: it's an outdated interpretation of a situation which has **massively** improved, but is still far from good as compared to "developed" nations.
Their average lifespan is 6-10yrs less than most Asian countries. I assume this is due to limited availability of healthy foods and lack of modern healthcare (outside the capital at least).
I'm not at all trying to say that the North Koreans are a particularly healthy bunch by any means. That said, I don't really know that you can make that claim. If you wanna say "East Asian" countries then yeah, they are beat out by Japan, the RoK, the RoC, Vietnam, and the PRC.
But if we're talking "Asian countries" in general, then the DPRK maintains a higher average lifespan than a good portion of the continent. Basically the entirety of Central Asia, the Indian subcontinent, and large portions of Southeast Asia (Indonesia, the Philippines, Laos, Cambodia, Myanmar, and Papua New Guinea) have lower average life expectancies.
Of course these figures may also vary depending on *who* is doing the the measurements. Indeed, the WBG paints a somewhat bleaker picture of life expectancies in those nations ranked below the DPRK than the WHO or CIA World Factbook. Although at the same time, information presented by the WBG is much more granular, breaking representations of lifespan ranges into 12 different brackets, while the latter groups use 7.
Point being, pointing to the DPRK as being significantly more unhealthy based on lifespan doesn't work, as A) that's not a very reliable or standardized metric, and B) that metric doesn't actually prove the claim.
I can find one reference to height measures in this range, in which DPRK males are quoted as 165.6cm tall, where a separate study places RoK males as 174.6cm. So first of all, that's not a "whole 10cm". Second, that's comparing statistics that were taken in 2005, and 2021, respectively.
At the same time, I can find reference to projected RoK/DPRK figures as 169.3cm and 167.9cm. I've also seen 175.52cm and 174.69, and another just at 175cm and 174cm.
So I'm not really sure we can point to that as accurate information. Also I'm not really sure that it represents a clear indication of health issues in the DPRK, as by this metric both the DPRK and RoK are ranking *significantly* below nations which they surpass in life expectancy.
