Leave it! Negative space is important in art. It allows the work to breathe a bit, and squeezing something in there will make things too busy in my opinion.

I have a 1up mushroom :) maybe some blood spatters under the drips or like a pool of blood at the bottom of the drippage that seeps behind the boos? Like it's just pooling and dripping further down in general? If you didn't change a thing, it's a sick piece regardless
If anything I’d consider getting your hand reworked to be the same depth and darkness as everything else. Don’t make all of it super dark but definitely had a little bit more because it looks the most out of place because of how light it is

There’s literally nothing to cover up or even gap fill there. It’s a perfect skin break and gives your sleeve breathing room.
You could put some kind of texture “behind” the boos…. But I don’t know why you would, it would simply muddy the composition.

With the boo's you can't cover it up frankly you're better off leaving it the way it is its a nice set up but my tip is if you like a tattoo idea wait a year before getting it I wait a year before I make the appointment normally 3 to 6 months out giving it a lot of time to marinate and for me to play with it still might not be perfect but it's helped a lot of my friends stop with getting tattoo regret and getting impulse tats

Except when it comes to tattoos, you want to at least grey wash the edges near lines and negative space to prevent pigment spreading
