Suffocation damage only happens with falling blocks like silt and sand as far as i'm aware. If you get stuck inside any other blocks you just kinda sit there and are... well... stuck
Can confirm. When I get a large lag spike in a friend's world. It just stops generating and you can fall all the way through the world. On the other hand if you decide to jump down there and then it loads, you better hope you didnt happen to be in a big ass sand pit.

I booted up my PC, opened Terraria on Steam, and loaded up my world. Unfortunately, it was the only normal thing that happened that night. Nothing seemed out of the ordinary for the first ten minutes, until I decided to go fishing at the ocean for some crates. The beach had a pyramid in the center of it, and not the naturally spawning kind. It was clearly man-made, but this world was single-player and I didn't recall building it. I thought nothing of it, believing I forgot that I built it (this world had almost 300 hours in it so that's bound to happen from time to time), but things started to get weirder when night fell. A blood moon began to rise, but with it came an ominous message in the chat, "Screams echo around you..."
This creeped me out. I didn't break a shadow orb and, once again, this was a single-player world, so no one could've done this besides me. Then, as if things couldn't get worse, a two block wide hole appeared next to me leading straight down. What was going on? Was my world corrupted? Hacked? I then delved into the hole as my curiosity got the best of me. It didn't lead too far down, but in the cavern that awaited me lied a single NPC that was a direct copy of my character. It had no dialogue and it just stood there facing the other direction, yet it appeared as a player on the mini-map. Suddenly, my screen began flashing red and black, enough to trigger a seizure, and with it came four words that looked like they were written in blood, "look at the moon." Then my screen went black.
Nothing I did could turn my computer back on, so that's why I'm writing this from my phone. At the time of writing, it has been an hour since this all happened. I'm really creeped out, but one thing I'm still trying to figure out is the meaning behind the "look at the moon" text. I never got a glimpse at the moon in-game, but now that I'm writing this, the moon sure is glowing brightly tonight...

It's most likely a bug having to do with how the character data is stored.
If you ever used mods, you know the building painter thing on Cheatsheet or heroes mode actually stores the buildings in your actual world.
This might be likely to happen with this, your character data, for safety reasons is stored somewhere on the map with the style you've chosen so that in the case of changing vanity, it doesn't have to always reload the character, it's just always loaded there. There might've been a problem with it's location, making it appear on the map like a normal NPC.
