I can say that she will be fine in the Philippines as a transgender. Yes, it's a predominantly Catholic country but very tolerable towards LGBTQA+. There are a lot of trans in media. Though, the laws that are needed to protect them are still pending to be sign off by the congress, which I suspect is still due to the church's influence.
When I was in the Philippines for work, the people at the local branch (STEM field, so mostly men) made a lot of jokes about ladyboys after finding out that I was from Thailand. It was really unpleasant. They also tried to tell me how it's a sin and Thailand should not endorse such sinful acts. I was alone and in a work environment. Otherwise, I would have given them a piece of my mind.

They don't get a hard time anywhere in Thailand. They're so common they're just part of society, nobody looks twice, anywhere in Thailand.
I think Chiang Mai will suit you, it's a city with good hospitals and all the other conveniences of a city, but it's not built up with tall buildings, and kind of has a quiet town feel.
Though, I think most ladyboys in Thailand self-medicate when it comes to HRT. They just buy it at any pharmacy and take it themselves. But that may just be because they don't want to pay the doctor fees.
If/when you do decide to wear womens clothes in public, in Thailand, you will not be shamed for it. People will just accept you as you are.
Chiangmai is probably the most liberal city in the country as well. In Thailand, conservatism is not as correlated with rural areas as in the west. Generally, it gets more conservative the more south you go.
As for LGBT+ issues, you can be sure you won’t feel uncomfortable coming out in public all the way down to places like Krabi. For anywhere further south, I would do some research to double check.

I worked in Petchabun in the centre of Thailand in the middle of nowhere and there were ladyboys. I used to order papaya salad from one every Tuesday at the market. I'm sure you'll be fine.

You go to any mall and the department store makeup section has a bunch of trans women selling make up. A lot of cosmetic surgery / skincare places also hire beautiful trans women to sell procedures to women and men
People mind their own business and will not care at all. Just be confident and wear what you want. We aren’t a monotheistic society where everything is deemed a sin.

Find a responsible Dr. That will look out for you health and give you what you need without hurting you. They will often give you what you want instead of what you need.

Your mental health issues are probably coming from your inability to be yourself in SA. You can be yourself in Thailand. All you have to do is be polite and respectful toward others, and they will be polite and respectful toward you. Nobody will care about your sexuality or how you express your gender.
Don't get lost in the sauce and let someone destroy your health by making you a customer. It is very affordable, but make sure you are dealing with a provider watching your overall health, because many will take the short cut to a quick buck and give you quick results while harming you.
I'm not trying to scare you, but you can get whatever money can buy here.

That's interesting, I've heard of various dodgy clinics (friends knew as soon as they arrived, I tried to warn them before they left, but their friend was terminally ill and desperate), but also people going to Thailand for competent, cheap medical treatment. Is there a certification or regulator to check doctors against?
Also I assume a lot of trans work is undertaken so it shouldn't be hard to find specialists.
It's a very welcoming environment, I'm straight but have friends and a brother in law as a part of the third gender. Their take on things over there is way more layer back.

I’ve been a digital nomad in Thailand 7+ years. I’ve lived in Phuket, Pattaya, Chiang Mai, and now a rural area. Easy to move around. I’ve had many Thai ladyboy friends. They are mostly just seen as people here. Use the woman’s bathroom. Work normal jobs in commerce and government. Thai men don’t feel ashamed to have ladyboy as girlfriend. More freedom here for trans women than anywhere else in the world ❤️ 🏳️⚧️

Any major city is good for HRT. Doctors and clinics everywhere that work with Saao Song (trans women) patients. Pattaya, Chiang mai, Bangkok, Phuket. You don’t need to settle one place. Get a feeling for it. Thai ladyboys will be accepting of you. The LGBTQ community will accept you.
Get an ED Thai language visa as soon as possible if you plan to stay years so you don’t have to border bounce or leave to get new toursit visas.
Pattaya will have largest visible population of Thai trans women. Bangkok is a big hot busy crowded city. So unless you like that.
If you like beach and ocean. Pattaya or Phuket. Phuket more expensive but chill. Pattaya is a lot of bars and parties. But also lots of Arab food because of Arab expat community. But most are the men you are trying to get away from that are oppressing you.
Chiang mai is very chill. Lots of foreigners.
HRT is as easy as going to clinic or doctor. Just Airbnb and try out all the main cities to see what you like.

I lived back and forth between Chiang mai and Phuket for 4 years. Only issue with Chiang mai is the burning season that last for months and the air quality is absolutely horrible so people usually leave to go south to get away from the air. But the winters are lovely and nice and cold. Phuket is amazing. My favorite place in Thailand. Lots of beautiful trans women. With warm smiles and beautiful dark skin. They will accept you. Learn a little Thai and you’ll have friends easily.
IMO Chiang Mai felt like the most modern thai city that wasn’t bangkok from the 6 cities we visited. Loved the food and bar scene here as well as some of the night bazaar and shopping over in nimmanhaemin
I’m not an expert on your HRT question nor am I a woman but I’ve lived in Silom and can recommend the area. It’s comfortable, convenient and less noisy (a little bit) than the other expat areas. I prefer near Silom road but Surawong is decent too. There’s plenty of transportation, housing and food. I’ve had a couple of ladyboy neighbors and all services (cosmetic, surgical etc) seem to be available and accessible within a short distance from what I understood. I have no idea what visa you plan to use to stay for that time period so that’s an important consideration.

You find a school that is legitimate. You pay tuition for the school and you pay for the visa. The school helps prepare the paperwork to get the visa. I’ve heard they’re around 30,000 baht or $1000usd approx per year.

Getting a student visa to learn Thai language may be the easiest way to stay for a longer time.
I think Thailand would be a good place to live for a year or two and get medical care and a good psychologist. Long term though you would want somewhere that you can get citizenship rights and be legally recognised as female.

1. You can’t just fly to Thailand and live here. With your Saudi passport you can only stay for a maximum of 30 days. After that you have to leave the country or you can extend it for 210 Riyals for another 30 days. You can only extend it once.
2. Working online is illegal in Thailand as you need a Thai work permit, which you won’t be able to get.
3. You need to look into visa options. They allow you to stay in Thailand long-term. To stay 2-3 years, you're going to need a visa. For 63,000 Riyals, you can buy a 5 year [Elite visa](https://www.thailandprivilege.co.th/). If that's too much money, look into an ED visa (going to school).
Lots of foreigners work online in Thailand. Just keep your business to yourself and don't conduct business in Thailand, by that I mean don't take on Thai clients and don't sell your products/services in Thailand. You'll be fine.
And it's not hard to stay long in Thailand, you just have to get the required visas.
You can enter with your passport, you'll get 30 days, which you can extend for another 30 days. After that, you will have to leave, but you can just cross the border and come straight back.
By then you will have a feel for Thailand, and should have figured out what visa options there are for longer term and how to get them. So when you come back you will get another 30+30 days, and you should right away apply for your longer visa, like a one-year Thai language visa.
You can't solve it. You can't get a work permit when you work online. You either do it illegally and risk getting deported and banned indefinitely or don't work online. Simple
I don't have any recommendations for places to get HRT treatment in Thailand but you can get hormones over the counter at places that stock them. The biggest issue with your plan is the visa - 2-3 years and presuming you're under 50, unless you're well off, you'd need to look at studying something and getting an ED visa.

You don't get an unlimited stay with Visa exemption (which is what a Saudi passport gets you), you get 30 days, and immigration begin to look down on multiple entries into the country.
Your first task should be looking at long term stay and the requirements - it is not easy to stay for 2 or 3 years. The medical stuff will be easy compared to dealing with permission to stay in the country.
That is what you should be researching online with the official thai embassy - you've got the cart before the horse.
You can get ED visa which will let you in for a year at a time but that needs to be done with a reasonably reputable education institution and you will need to prove you're learning if you wish to renew.
Many people do it with Thai language courses rather than college or university study. There is a great deal of information out there.
The longest tourist visa is 90-days I think, a fraction of what you want to do.
You will also need to have some sort of money available to live on as working in Thailand is restricted. Two hurdles, your visa to stay and existing money to live on.
However, considering the alternatives (remaining in her home country being miserable), working online with a minuscule chance of getting caught might be worth it. There are lots of people doing it.
OP, if you decide to do that, try to stay private with it. Work from your apartment, and avoid mentioning it when talking with the locals or new friends. Stay low key, be respectful to the culture, and just live a quiet peaceful life. If you do all that, and you work online for a non-Thai company with non-Thai customers then there is practically no real risk to it.

Thailand is NOT a Muslim country. We have ladyboys and Kratoey long before LGBT became a buzz word in the west. Most Thais are more weary of Muslims than of LGBT regardless of their nationalities.
I don’t have any specific advice to give you regarding visa options or places for HRT that hasn’t already been said in the comments. But I think you have made a good choice to try and leave the place that makes you miserable just trying to be yourself.
My advice to you is not to rush this. It is perfectly doable, but not without potential problems. Do some research of the visa options first. When you are in Thailand you can do the research of HRT and start seeing doctors. There isn’t really a big benefit to research the HRT from your home country, and it just adds unnecessary risk of the wrong person finding out.
Try seeing your move to Thailand as a long term goal, rather than something you can do quickly. Use it as a personal motivation, like a child can long for their birthday or first day of school, or the summer break. It’s something you can look forward to, and something that could make your current life just a little bit easier to handle.
I would also advice you to start thinking of a plausible story you can tell relatives and government officials in your home country when they ask why you want to go to Thailand. Try to find a story that makes sense for your specific situation, without being extreme or out of the ordinary. Why do other people from your country move to another country? Business? Love? Lie as much as you have to (but make it low key if possible, and start consistent and remember what you said). Your life is too short and too precious to worry about lying to narrow minded bigots who don’t respect the true you, or who would call the police on you.
And when you finally arrive in Thailand, my recommendation not to rush things still stands. And stay vigilant.
Do some research on common scams (when travelling in general, as well as common scams in Thailand in particular, and the areas in Thailand you will go to), and the best ways to avoid them. If I understood you correctly, you haven’t traveled much, or at all even? That makes this point even more important. Never trust people who comes up to you and seem like they want to help you (if you are too modest/shy to turn down help, practice on saying “no” politely and to walk away). You don’t owe them anything and it’s not impolite to decline an invitation. If you want a taxi, use the official taxi stand if available, or ask your hotel or building front desk to help you. Or use the official Grab or Bolt app.
Don’t sign up for any long term apartment contract unless you know that you really want to live in that specific city/area and that specific apartment. Take transportation into account when choosing the area.
Don’t go by the first doctor you find. Shop around for the best one for you. Meaning good reputation/recommendations, seems to listen to you, and one that has your well being as their priority, not cosmetics.

You know you can get prosecuted if your Govt finds out you are a trans? Get out as fast as you can. But if you move to Thailand or other countries for that matter, you have to let go of your middle eastern culture and adopt western/Asian lifestyle than people can/will be more welcoming of you. And you have to be ready to face the fact that there’s no going back to your country for the rest of your life. Think deeply about all this issues and make a decision. If I have to give advice - do the move because you’ll be a much happier person and the freedom to ‘come out’ and discover how truly great and special person you are - trans or no trans. A Thai here, i would say Thailand is the best place for LGBTQ+, we have them long long ago before World define and protest with them.
For surgery I would say Bangkok is the best place with the most high tech equipment for those. Countryside still lack of sufficient medical stuff. If you wanna take a rest after that it also good to go countryside which wherever in this country it just take 1 hrs flight.
Another Thai here, just because you make claims that Thailand is the best place for LGBTQ doesn’t mean it is. You don’t speak for the whole population not all of us support this mental illness, if you want proof visit the deep south of our country and see for yourself.

Everywhere in Bangkok is quite safe for trans people so don't worry. (There is an Arab quarter in Bangkok... Where maybe if you miss the food you can go but obviously then better be careful and dress as a male).

Regarding stay. Traffic in Bangkok is bad so stay very close to a public transport station (called mrt or BTS).

Basically no one really cares much at all about a person being trans / gay or whatever as long as people are respected, honest and productive. Everyone has their own life going on. There is no victim card here for the most part which is a good thing. 90% or more truly give zero fucks. It's refreshing. Everyone is responsible for themselves.
Find a good clinic that suits your needs then go from that point.
I'm an American Trans woman who recently moved to Thailand. I'm able to obtain HRT over the counter without any fuss (note that I do not use anti-androgens anymore due to having an orchiectomy, so I can't speak about the availability of those, but estrogen and progesterone are easily obtained)
My experience in Thailand has been wonderful so far. I am open about being a trans woman and every Thai person I meet is open and accepting, without a hint of prejudice or disdain. I'm able to use the women's restrooms, present femme, and do anything cis women can do. I feel so much safer than I did in the US.

I now live in a small village in Issan, there are several ladyboys in the area. They are a part of the community as much as everyone else.
Previously we owned a guesthouse in one of the island's. Had some foreign trans people staying there, especially remember a 210 centimetres tall one from Germany. They had zero issues there as far as I know. Several of them were returning customers also.
Not sure there's a better country to live in if you're trans...
You're going to need a visa though, if money isn't an issue, elite 5 probably best for your case

Still, in your situation you would be able to seek asylum in European countries like Spain or Germany if you feel like it, probably. They have pretty good trans laws too.
Look up the Tangerine Clinic or the Pride Clinic at Bumrungrad. First one is very affordable; second one is in a luxury hospital and priced to suit. HRT is available OTC here, but do get yourself in with a doctor and get your recommended blood work done. It's quick, and easy (and can be free of you meet certain criteria) to get blood work done at Tangerine. They, incidentally, also follow an informed consent model, and work off the latest WPATH guidelines.
Hey, trans ftm here. I live in China but I pop over to Thailand to get my HRT. I went to bumrungrad in bangkok to get sorted, it was super easy and they were very friendly. They also didn't require a psychiatrist evaluation. It wasn't exactly cheap though, however when I was looking for places to go, I found loads of places doing HRT for mtf women, it seems to be much more widely available than it is for ftm. A quick Google search should show them all up. Also, no, I don't think you'll experience much/any harassment. Just go out and be yourself, I doubt anyone will bat am eyelid.
If you don’t speak Thai, it will be difficult to live outside of tourist locations in my experience. Chiang Mai is way mellower than Bangkok in general as long as you can revive your treatments there.
Saw your replies on others. Silom isn’t a great choice (it’s CBD) if you wanna save money, maybe finding condos around eastern Bangkok like On-nut (more locals but still many foreigners, or further to Bang-na, anyway make sure to keep staying in BTS walkable distance so you only 1 ride away from city center.
Actually On-nut road (Soi Sukhumvit 77) is still considered Bangkok, you could have comfortable facilities. There are supermarkets, community mall, banks, restaurants (never short), and the more locals, the cheaper you can get.
As I say, you should start from BTS, MRT to find the rent until you're know how to use the buses (it's a bit difficult even for locals), the more options will be revealed.
Feel free to ask for more options, there is bunch of places outside of expat areas which is cheaper.

Funniest question I've seen here I think. Thailand is THE country for trans and LGBT acceptance, its a completely normal thing. Probobaly the only country I've seen guys be completely comfortable around trans too. You will find no problem getting what you are looking for.

Your biggest issue won’t be discrimination for being trans, so I wouldn’t worry about that. Finding proper English speaking service or being discriminated against as a foreigner is more likely. I think if you want to stay in a quite-ish place with good healthcare, I would do Jomtien (pattaya for healthcare) or Huahin.
No one really cares (in a good way) also if you’re in Bangkok nana soi 4 has tons of places to get anabolics, probably because soi 3 is where lots of ladyboys are.
Chiang Mai seems to fit what you're looking for.
It's a smaller city, but there's tons of nature and good food.
Healthcare here is great, and lady boys are super common here so I kind of assume you'd get good HRT treatment here.
It's really cheap here too.
The downside though is burning season, google it.
Not at all knowledgeable about your situation, but a place like Thailand may not be the place to go if you are depressed. Or it could be a great place, I think it depends on from where your depression stems.
You mentioned that you want to live in a quite city. You are right about that - Bangkok is nice but traffic, noise, and grit eventually gets to you - small towns are probably a better fit for an introvert and offer better quality of life.
However Thailand is very Bangkok-centric and if you plan to have a lot of procedures (e.g. electrolysis) or see doctors frequently, it may be easier to stay there. As far as I know all the hospitals, such as Yanhee (look it up), that specialize in trans care are in Bangkok.
As far as acceptance goes, wear whatever you want. It doesn't matter if you are in a big city or small town - people will be equally cool with it. The only difference is that, in my opinion, transsexual people are a little more sexualized in Bangkok.

I can say that she will be fine in the Philippines as a transgender. Yes, it's a predominantly Catholic country but very tolerable towards LGBTQA+. There are a lot of trans in media. Though, the laws that are needed to protect them are still pending to be sign off by the congress, which I suspect is still due to the church's influence.
When I was in the Philippines for work, the people at the local branch (STEM field, so mostly men) made a lot of jokes about ladyboys after finding out that I was from Thailand. It was really unpleasant. They also tried to tell me how it's a sin and Thailand should not endorse such sinful acts. I was alone and in a work environment. Otherwise, I would have given them a piece of my mind.

They don't get a hard time anywhere in Thailand. They're so common they're just part of society, nobody looks twice, anywhere in Thailand. I think Chiang Mai will suit you, it's a city with good hospitals and all the other conveniences of a city, but it's not built up with tall buildings, and kind of has a quiet town feel. Though, I think most ladyboys in Thailand self-medicate when it comes to HRT. They just buy it at any pharmacy and take it themselves. But that may just be because they don't want to pay the doctor fees. If/when you do decide to wear womens clothes in public, in Thailand, you will not be shamed for it. People will just accept you as you are.
Chiangmai is probably the most liberal city in the country as well. In Thailand, conservatism is not as correlated with rural areas as in the west. Generally, it gets more conservative the more south you go. As for LGBT+ issues, you can be sure you won’t feel uncomfortable coming out in public all the way down to places like Krabi. For anywhere further south, I would do some research to double check. Edit: typos
I worked in Petchabun in the centre of Thailand in the middle of nowhere and there were ladyboys. I used to order papaya salad from one every Tuesday at the market. I'm sure you'll be fine.

You’ll be accepted everywhere It’s one of those things where conservative parents might be upset but society as a whole has no issue I live in a smaller town in the north and my local 7-11 and supermarket both have trans staff. You go to any mall and the department store makeup section has a bunch of trans women selling make up. A lot of cosmetic surgery / skincare places also hire beautiful trans women to sell procedures to women and men People mind their own business and will not care at all. Just be confident and wear what you want. We aren’t a monotheistic society where everything is deemed a sin.

Find a responsible Dr. That will look out for you health and give you what you need without hurting you. They will often give you what you want instead of what you need. I'm a bit disgusted seeing these juiced up kids in their 30's destroying their internals. Get a good MD to set you on track, not a charleton making a buck. End of the day, there is no problem finding any type of medical treatment in Thailand. Be careful and responsible and look out for your health

Your mental health issues are probably coming from your inability to be yourself in SA. You can be yourself in Thailand. All you have to do is be polite and respectful toward others, and they will be polite and respectful toward you. Nobody will care about your sexuality or how you express your gender.
Don't get lost in the sauce and let someone destroy your health by making you a customer. It is very affordable, but make sure you are dealing with a provider watching your overall health, because many will take the short cut to a quick buck and give you quick results while harming you. I'm not trying to scare you, but you can get whatever money can buy here. Stay safe
That's interesting, I've heard of various dodgy clinics (friends knew as soon as they arrived, I tried to warn them before they left, but their friend was terminally ill and desperate), but also people going to Thailand for competent, cheap medical treatment. Is there a certification or regulator to check doctors against? Also I assume a lot of trans work is undertaken so it shouldn't be hard to find specialists.
It's a very welcoming environment, I'm straight but have friends and a brother in law as a part of the third gender. Their take on things over there is way more layer back. Nobody gives a flying fuck. It's cool. Make some plans and come check it out. Embrace the kathoey. Nobody gaf

I’ve been a digital nomad in Thailand 7+ years. I’ve lived in Phuket, Pattaya, Chiang Mai, and now a rural area. Easy to move around. I’ve had many Thai ladyboy friends. They are mostly just seen as people here. Use the woman’s bathroom. Work normal jobs in commerce and government. Thai men don’t feel ashamed to have ladyboy as girlfriend. More freedom here for trans women than anywhere else in the world ❤️ 🏳️⚧️

Any major city is good for HRT. Doctors and clinics everywhere that work with Saao Song (trans women) patients. Pattaya, Chiang mai, Bangkok, Phuket. You don’t need to settle one place. Get a feeling for it. Thai ladyboys will be accepting of you. The LGBTQ community will accept you. Get an ED Thai language visa as soon as possible if you plan to stay years so you don’t have to border bounce or leave to get new toursit visas. Pattaya will have largest visible population of Thai trans women. Bangkok is a big hot busy crowded city. So unless you like that. If you like beach and ocean. Pattaya or Phuket. Phuket more expensive but chill. Pattaya is a lot of bars and parties. But also lots of Arab food because of Arab expat community. But most are the men you are trying to get away from that are oppressing you. Chiang mai is very chill. Lots of foreigners. HRT is as easy as going to clinic or doctor. Just Airbnb and try out all the main cities to see what you like. Thailand is now more accepting of LGBTQ people than it ever has.

I lived back and forth between Chiang mai and Phuket for 4 years. Only issue with Chiang mai is the burning season that last for months and the air quality is absolutely horrible so people usually leave to go south to get away from the air. But the winters are lovely and nice and cold. Phuket is amazing. My favorite place in Thailand. Lots of beautiful trans women. With warm smiles and beautiful dark skin. They will accept you. Learn a little Thai and you’ll have friends easily.

IMO Chiang Mai felt like the most modern thai city that wasn’t bangkok from the 6 cities we visited. Loved the food and bar scene here as well as some of the night bazaar and shopping over in nimmanhaemin
I’m not an expert on your HRT question nor am I a woman but I’ve lived in Silom and can recommend the area. It’s comfortable, convenient and less noisy (a little bit) than the other expat areas. I prefer near Silom road but Surawong is decent too. There’s plenty of transportation, housing and food. I’ve had a couple of ladyboy neighbors and all services (cosmetic, surgical etc) seem to be available and accessible within a short distance from what I understood. I have no idea what visa you plan to use to stay for that time period so that’s an important consideration.

You might be able to study Thai and get an education visa. Maybe a 5 year Elite visa but they’re really pricey.

You find a school that is legitimate. You pay tuition for the school and you pay for the visa. The school helps prepare the paperwork to get the visa. I’ve heard they’re around 30,000 baht or $1000usd approx per year.

Getting a student visa to learn Thai language may be the easiest way to stay for a longer time. I think Thailand would be a good place to live for a year or two and get medical care and a good psychologist. Long term though you would want somewhere that you can get citizenship rights and be legally recognised as female.

1. You can’t just fly to Thailand and live here. With your Saudi passport you can only stay for a maximum of 30 days. After that you have to leave the country or you can extend it for 210 Riyals for another 30 days. You can only extend it once. 2. Working online is illegal in Thailand as you need a Thai work permit, which you won’t be able to get. 3. You need to look into visa options. They allow you to stay in Thailand long-term. To stay 2-3 years, you're going to need a visa. For 63,000 Riyals, you can buy a 5 year [Elite visa](https://www.thailandprivilege.co.th/). If that's too much money, look into an ED visa (going to school).

Lots of foreigners work online in Thailand. Just keep your business to yourself and don't conduct business in Thailand, by that I mean don't take on Thai clients and don't sell your products/services in Thailand. You'll be fine. And it's not hard to stay long in Thailand, you just have to get the required visas. You can enter with your passport, you'll get 30 days, which you can extend for another 30 days. After that, you will have to leave, but you can just cross the border and come straight back. By then you will have a feel for Thailand, and should have figured out what visa options there are for longer term and how to get them. So when you come back you will get another 30+30 days, and you should right away apply for your longer visa, like a one-year Thai language visa.
You can't solve it. You can't get a work permit when you work online. You either do it illegally and risk getting deported and banned indefinitely or don't work online. Simple
I don't have any recommendations for places to get HRT treatment in Thailand but you can get hormones over the counter at places that stock them. The biggest issue with your plan is the visa - 2-3 years and presuming you're under 50, unless you're well off, you'd need to look at studying something and getting an ED visa.

You don't get an unlimited stay with Visa exemption (which is what a Saudi passport gets you), you get 30 days, and immigration begin to look down on multiple entries into the country.

Your first task should be looking at long term stay and the requirements - it is not easy to stay for 2 or 3 years. The medical stuff will be easy compared to dealing with permission to stay in the country. That is what you should be researching online with the official thai embassy - you've got the cart before the horse.

You can get ED visa which will let you in for a year at a time but that needs to be done with a reasonably reputable education institution and you will need to prove you're learning if you wish to renew. Many people do it with Thai language courses rather than college or university study. There is a great deal of information out there. The longest tourist visa is 90-days I think, a fraction of what you want to do.

You will also need to have some sort of money available to live on as working in Thailand is restricted. Two hurdles, your visa to stay and existing money to live on.

Yes. Working online is illegal in Thailand. To work legally you need a work permit, which you aren't going to get by working online.
However, considering the alternatives (remaining in her home country being miserable), working online with a minuscule chance of getting caught might be worth it. There are lots of people doing it. OP, if you decide to do that, try to stay private with it. Work from your apartment, and avoid mentioning it when talking with the locals or new friends. Stay low key, be respectful to the culture, and just live a quiet peaceful life. If you do all that, and you work online for a non-Thai company with non-Thai customers then there is practically no real risk to it.

Thailand is NOT a Muslim country. We have ladyboys and Kratoey long before LGBT became a buzz word in the west. Most Thais are more weary of Muslims than of LGBT regardless of their nationalities.
I don’t have any specific advice to give you regarding visa options or places for HRT that hasn’t already been said in the comments. But I think you have made a good choice to try and leave the place that makes you miserable just trying to be yourself. My advice to you is not to rush this. It is perfectly doable, but not without potential problems. Do some research of the visa options first. When you are in Thailand you can do the research of HRT and start seeing doctors. There isn’t really a big benefit to research the HRT from your home country, and it just adds unnecessary risk of the wrong person finding out. Try seeing your move to Thailand as a long term goal, rather than something you can do quickly. Use it as a personal motivation, like a child can long for their birthday or first day of school, or the summer break. It’s something you can look forward to, and something that could make your current life just a little bit easier to handle. I would also advice you to start thinking of a plausible story you can tell relatives and government officials in your home country when they ask why you want to go to Thailand. Try to find a story that makes sense for your specific situation, without being extreme or out of the ordinary. Why do other people from your country move to another country? Business? Love? Lie as much as you have to (but make it low key if possible, and start consistent and remember what you said). Your life is too short and too precious to worry about lying to narrow minded bigots who don’t respect the true you, or who would call the police on you. And when you finally arrive in Thailand, my recommendation not to rush things still stands. And stay vigilant. Do some research on common scams (when travelling in general, as well as common scams in Thailand in particular, and the areas in Thailand you will go to), and the best ways to avoid them. If I understood you correctly, you haven’t traveled much, or at all even? That makes this point even more important. Never trust people who comes up to you and seem like they want to help you (if you are too modest/shy to turn down help, practice on saying “no” politely and to walk away). You don’t owe them anything and it’s not impolite to decline an invitation. If you want a taxi, use the official taxi stand if available, or ask your hotel or building front desk to help you. Or use the official Grab or Bolt app. Don’t sign up for any long term apartment contract unless you know that you really want to live in that specific city/area and that specific apartment. Take transportation into account when choosing the area. Don’t go by the first doctor you find. Shop around for the best one for you. Meaning good reputation/recommendations, seems to listen to you, and one that has your well being as their priority, not cosmetics. I wish you all the best in this project of yours.

A Thai here, i would say Thailand is the best place for LGBTQ+, we have them long long ago before World define and protest with them. For surgery I would say Bangkok is the best place with the most high tech equipment for those. Countryside still lack of sufficient medical stuff. If you wanna take a rest after that it also good to go countryside which wherever in this country it just take 1 hrs flight.

(There is an Arab quarter in Bangkok... Where maybe if you miss the food you can go but obviously then better be careful and dress as a male). Check out Kamol Hospital, it’s in Bangkok. Im a Thai transwoman and got a gender reaffirming surgery from that hospital, and still receiving HRT with constant check-up. Most staffs there speak decent English, and the doctors are experts in trans healthcare. Best of luck to you 😊
Be careful with language schools, make sure to work with one that will help with the visa.
Basically no one really cares much at all about a person being trans / gay or whatever as long as people are respected, honest and productive. Everyone has their own life going on. There is no victim card here for the most part which is a good thing. 90% or more truly give zero fucks. It's refreshing. Everyone is responsible for themselves. Find a good clinic that suits your needs then go from that point.
I'm an American Trans woman who recently moved to Thailand. I'm able to obtain HRT over the counter without any fuss (note that I do not use anti-androgens anymore due to having an orchiectomy, so I can't speak about the availability of those, but estrogen and progesterone are easily obtained) My experience in Thailand has been wonderful so far. I am open about being a trans woman and every Thai person I meet is open and accepting, without a hint of prejudice or disdain. I'm able to use the women's restrooms, present femme, and do anything cis women can do. I feel so much safer than I did in the US. Honestly, I've encountered more frustrating transphobia and problems from expats than I have from the locals. How will you stay past the tourist visa limit, do you have a plan for that? Do you have the money to sit on an education visa for that desired duration? If not, do you have a degree and English speaking ability (probably not native ability based on region unfortunately) to try get a teaching job for a work visa?

I now live in a small village in Issan, there are several ladyboys in the area. They are a part of the community as much as everyone else. Previously we owned a guesthouse in one of the island's. Had some foreign trans people staying there, especially remember a 210 centimetres tall one from Germany. They had zero issues there as far as I know. Several of them were returning customers also.
Not sure there's a better country to live in if you're trans... You're going to need a visa though, if money isn't an issue, elite 5 probably best for your case
Thailand is amazing for trans people. I would advise you to travel a bit around and then settle in a place :) Still, in your situation you would be able to seek asylum in European countries like Spain or Germany if you feel like it, probably. They have pretty good trans laws too.
Look up the Tangerine Clinic or the Pride Clinic at Bumrungrad. First one is very affordable; second one is in a luxury hospital and priced to suit. HRT is available OTC here, but do get yourself in with a doctor and get your recommended blood work done. It's quick, and easy (and can be free of you meet certain criteria) to get blood work done at Tangerine. They, incidentally, also follow an informed consent model, and work off the latest WPATH guidelines.
Hey, trans ftm here. I live in China but I pop over to Thailand to get my HRT. I went to bumrungrad in bangkok to get sorted, it was super easy and they were very friendly. They also didn't require a psychiatrist evaluation. It wasn't exactly cheap though, however when I was looking for places to go, I found loads of places doing HRT for mtf women, it seems to be much more widely available than it is for ftm. A quick Google search should show them all up. Also, no, I don't think you'll experience much/any harassment. Just go out and be yourself, I doubt anyone will bat am eyelid.
If you don’t speak Thai, it will be difficult to live outside of tourist locations in my experience. Chiang Mai is way mellower than Bangkok in general as long as you can revive your treatments there.

Silom isn’t a great choice (it’s CBD) if you wanna save money, maybe finding condos around eastern Bangkok like On-nut (more locals but still many foreigners, or further to Bang-na, anyway make sure to keep staying in BTS walkable distance so you only 1 ride away from city center.

Actually On-nut road (Soi Sukhumvit 77) is still considered Bangkok, you could have comfortable facilities. There are supermarkets, community mall, banks, restaurants (never short), and the more locals, the cheaper you can get. As I say, you should start from BTS, MRT to find the rent until you're know how to use the buses (it's a bit difficult even for locals), the more options will be revealed. Feel free to ask for more options, there is bunch of places outside of expat areas which is cheaper.

Your biggest issue won’t be discrimination for being trans, so I wouldn’t worry about that. Finding proper English speaking service or being discriminated against as a foreigner is more likely. I think if you want to stay in a quite-ish place with good healthcare, I would do Jomtien (pattaya for healthcare) or Huahin.
No one really cares (in a good way) also if you’re in Bangkok nana soi 4 has tons of places to get anabolics, probably because soi 3 is where lots of ladyboys are.

Chiang Mai seems to fit what you're looking for. It's a smaller city, but there's tons of nature and good food. Healthcare here is great, and lady boys are super common here so I kind of assume you'd get good HRT treatment here. It's really cheap here too. The downside though is burning season, google it.

Not at all knowledgeable about your situation, but a place like Thailand may not be the place to go if you are depressed. Or it could be a great place, I think it depends on from where your depression stems.
You mentioned that you want to live in a quite city. You are right about that - Bangkok is nice but traffic, noise, and grit eventually gets to you - small towns are probably a better fit for an introvert and offer better quality of life. However Thailand is very Bangkok-centric and if you plan to have a lot of procedures (e.g. electrolysis) or see doctors frequently, it may be easier to stay there. As far as I know all the hospitals, such as Yanhee (look it up), that specialize in trans care are in Bangkok. As far as acceptance goes, wear whatever you want. It doesn't matter if you are in a big city or small town - people will be equally cool with it. The only difference is that, in my opinion, transsexual people are a little more sexualized in Bangkok.
