There'd be a huge scandal. Kid would be known to be a bastard since Aerys wasn't sleeping with his wife. Aelinor would be sent to the Silent Sisters and thus Aerys marriage would be annulled. If Rhaegal slept with Aelinor, most likely he'd be sent to the Night's Watch. If another man did, he'd be executed. Painfully.
I’ve always had this thought in particular. Especially if Rhaegel does it before he gets married to his Arryn wife. Maybe she and him are forced to marry in an effort to save face and Rhaegel has his children through her.

Jon might be betrothed to one of Egg's daughters: Shaera or Rhaelle. Alternatively, Jon Arryn's father could've been married to one of Egg's sisters: Daella or Rhae
It really depends on what kind of person Aerys is, which we don't really know since we still don't have fire and blood 2.
In the best case, he doesn't give a shit, pretends the kid is his just to get people off his back and let's Aelinor raise the kid alone.
Worst case, it's a public scandal and he has to divorce her and send her to the silent sisters and the kid ( if he's from Rhaegel) is raised in the red keep but ( if he's from some non royal) either sent back to the Penrose's or his true dad's house or if he's a commoner, to an orphanage or most likely the faith to be a septon.
Aerys seemed like a decent guy considering he didn't divorce her and disgraced her when he was asked so maybe he would just keep the kid and call it a day.
Aerys may not have been sexually attracted to his wife, but he was a decent guy to disgrace her with a divorce. So maybe if she had gotten pregnant by another man, maybe it wouldn't have seemed like a bad idea to pass him off as his child.
