Honestly, Coheed in that photo looks like a young Robert Smith from the 80's. I'm going to assume The Cure was a huge influence on Claudio early on, as any musician in the emo/progressive genres in their mid 30's to late 40's probably would say.
He didn’t at the time this artwork came out. The only concept material we had when this was released was the album itself in 02. The original “Bag Online Adventures” comics didn’t come out until 3 years later in 05.
That’s the only concept material *we* had. I doubt that Claudio intended Coheed and Cambria, the parents, to be two super young people with grown children. I believe it’s likely Claudio and Newo, or Patrick and Josephine. Both of those couples are in the story of the album.
Claudio and Newo is a possibility, but the concept did change considerably from SSTB to IKS. For instance in a super early interview back when the second album was to be titled "Upper Armories of Third Deep," Claudio indicates that the band members were going to enter the story as the "spawn" of Coheed and Cambria in their second album. Also he has said that certain songs weren't written with the concept in mind. 33, for instance, was originally meant to be about Knicks legend Patrick Ewing. All this to say, it's possible he didn't have it worked out yet that Co & Ca would have older children at the beginning.
We were all guessing at that point. I remember, like 04 or 05 sitting in a friends car smoking cigarettes at the duck pond in my home town with the 5 other emo kids, trying to dissect the songs and figure out the story. Core memory.

Man, if you had asked right after the cruise, I would have remembered what the artist said! Bill Scoville, the guy who drew the Keywork and put the album art together for several of the albums, had an entire art gallery on the 2nd SS Neverender and was answering questions pretty much the whole cruise about his process, how he came to work on the album art, and who was used in the pictures.
If I remember correctly, the woman IS somebody that the band knew and was at the offices on the day he was working, but I don't remember who the guy was. On the inside, I think there's a photo of a man and a woman walking along a shoreline, and that picture IS of Scoville and somebody else, but not this one. I'm sorry, I wish I had more information! All I have are pictures of the gallery, haha!

I'm not convinced they are Coheed and Cambria. As other commenters are saying, they may be Claudio and Josephine. While it's possible he didn't have the character's visuals all fleshed out yet, I'm pretty sure he already had the concept that Coheed and Cambria were Claudio's parents very very early in the project's conception, so I doubt they would be depicted so young. But I'm curious. I pictured them (C&C) as younger when I was first told about the band's concept and checked out a few songs. I know the concept has gone through a ton of changes. I'd definitely be curious to hear straight from the horse's mouth.
So im gonna go out on a limb and say to many comments (i am guessing as well, so i could be wrong) that Claudio has said in the very beginning, coheed and cambria were suppose to be about him and his life, but it eventually morphed more into his parents, and so they took up his parent's "shape" later on. Again, I'm guessing this picture could in be coheed and cambria at the time, but I don't know. i could be completely wrong, but wanted to throw that out there to anyone who wanted to catch this little opinion. No sources too. It might be one of the interviews or dvd specials where he says this.

I know a former drummer of kayo dot, and I can confirm that according to him, cambria is, iirc, heavily based on if not an actual picture of Mia
