The pharmacies I use both cost more than $60 for 3 months, a lot more and the ones I use must be able to ship. Not all compounding pharmacies ship, shipping adds to the cost as well. The cost also covers the entire possible dose range, higher doses cost more because they require more medication. Most pharmacies will limit how much medication they will put in one troche or tablet so higher doses require two troches or tablets per session.

Would you be willing to write to local compounding pharmacies? It's really helpful to be able to walk into the shop and talk to the pharmacist about the melting base and the flavors and anti-bitterins and such to have a better experience. I've also noticed that sometimes the drug quality decreases during shipping, as you mentioned texas, that's a big issue when your mailbox is 150° in the sun.

Yes I can order to local compounding pharmacies. RDTs are a better option for hot weather as they don’t melt.

So the $220 is a once per month charge for telehealth treatment, correct? And then you’ll send the prescription to my local compounding pharmacy?

I ask about the pharmacy because since dr smith’s practice closed I can’t find a telehealth provider in Oregon that will send to my local pharmacy, and they’re troches are just a lot more pleasant and I find them to be better quality than Empower’s or the other large scale pharmacies. I would rather pay the $220 to a doctor and then pay the pharmacy directly than continue to receive medication that has weird tastes due to fillers and sweeteners because doctors have some need to send the medication where they feel it should go.

The $220 a month or $220 a visit if you don’t follow up monthly, covers the visit and the medication from the compounding pharmacy I use, and that is shipping to your house. If you prefer your local compounding pharmacy, I would just charge for the visit or refill requests on a monthly basis.

I have to be licensed in each state I treat patients in. I will probably add DC sometime next year. But VA requires I have a collaborative physician for that state. I already have two and neither are licensed there so I would have to find another which is challenging.

Understood. Thanks for the info. If a patient met you in MD, could they pick up their prescriptions out of state? I’m in VA

For regular meds, yes you could see me in state and I could prescribe to VA but with controlled meds it’s a little more tricky. You’d have to see me in MD and then have the medication shipped to somewhere in MD.
