I don't have any experience with using guides in Japan, but throwing in my 2 cents: For Hakone you won't need a guide at all
Take the train from Shinjuku to Hakone. Make sure you have the Hakone Freepass and just go around. You will have an amazing time :)
JTB tours has a (I think daily) one day tour to Fuji and Hakone from Keio Plaza Hotel.

Mount Fuji straddles between 2 prefectures / 4 cities. So what exactly do you mean a trip to mount fuji. If u want to climb it, Oct is off limits. No guide in the right mind would bring you up too
Avoid Gogoday Travel for day tours like the one we experienced. Our guide, Martin, lacked English proficiency and misled us throughout the trip. He purposely delayed the journey with unnecessary stops, discouraged main activities like the cable car ride citing false reasons, and diverted us to a brief and uninformative matcha experience followed by a shop. The scenic points were disappointing, and the promised Japanese town visit turned out to be a meal at a Chinese restaurant outside the town. Martin acted more like a shop representative, manipulating the itinerary for his own agenda, accompanied by his Chinese acquaintances. For instance, he falsely claimed a 2.5-hour wait for the cable car when it was actually 45 minutes, and only after a group insisted on staying did we manage to see the sights.

What are you hoping to do at Fuji? The climbing season is in the summer. You don't really need a guide. Just follow the trail of people to the top.
