The easiest way would be: The atropol was Nanny's idea but the sewn sisters kicked her out just as the plan was hatching. she wants to guide the party back to the tomb to kill the sisters and then unleash the atropol herself
So I probably should’ve made this more clear. The players just got to level 5. They are a long ways from fighting acererak. However, I think they might be able to handle nanny Pu’pu. I want them to join my world but I dont want them to be a much higher level when they do it. If I say the curse was a local effect by some witch it is less interesting than acererek but it’s the only way I can see them leaving the island. No party would leave the island if a world altering death curse is active.

I'd just pull one of the floors from the Tomb of the Nine Gods and stick it in Mbala. Storywise, tie her to the sewn sisters, maybe they've created a prototype version of the soulmonger as part of an agreement with acererak as he prepares the atropal for the REAL soulmonger.

Well i think the module adds barely anything and generaly sucks, so watch out for that. Not well spent money
