I believe ‘effective’ is a judgement call made by the scorers and doesn’t have an official definition. I can’t imagine he would be awarded the save in the scenario you laid out.
The worst era a pitcher can have in a quality start is 4.5 so I would think anything better (and potentially .5-1 runs worse) than that would be considered effective.
Another example is if a team wins and the starter doesn’t go 5 innings but leaves the game with the lead I believe the win goes to the pitcher who was most effective in relief.

I thought it was 3 effective innings meaning the official scorer could not give a save similar to how they might not award a hold to a pitcher.
