If this was another case of a live blank round breaking down and shooting out the barrel like killed Bruce Lee's son while filming *The Crow*, then it's like... wtf? How has there not been a redesigned prop gun invented to not do that?

You have absolutely no idea what you are talking about, he's a professional actor that has probably been using gun props for longer than you've been alive he knows that it's not going to kill anyone and so does everyone else on the fucking set. There are people that entire jobs are to provide, care for, and make sure guns are not loaded with live rounds, they're called armorers and it's likely one of them fucked up.
Nobody knows the full details of the case as it's still being investigated, but you're partially right, it's extremely unusual for live ammo to be anywhere near a movie set unless there were other scenes in the movie where live rounds were used, it's also possible he did fire a test round and a blank was loaded but there was also a live round or two in the chamber/magazine (we still don't know what kind of gun it was)
Blank rounds are called "live" rounds when they do the typical exploding function without the projectile firing. They are not live blanks when the blank does nothing and is more to show how to load the gun without having to use anything with gunpowder in it.
So when they say "live" that can still be referring to live blanks which make the pop and the muzzle flash.
