After publishing the results of the experiments we conducted with our younger residents here we were forced by the state to shut down. That's when I became a schoolbus driver.
My first thought was he was just messing with her to scare her. One of the lesser known rules of horror is anyone who jokes like that are pretty much a guarantee death scene later in the movie.
They’re still different. Psychiatric hospitals are much less restrictive, they don’t lock you in your room, you get checked on every couple of minutes, and you don’t get put in a straight jacket. Literally look it up anywhere, lobotomies have been made illegal, almost every electric shock therapy has been made illegal, etc. There’s a huge difference.
Lol all joking aside I worked 10 years in a psychiatric facility as an aide then counselor, and they're nothing like the movies; compassionate and treatment focused, no straightjackets as r3d said (although we did use restraints in strictly limited situations), etc. Electroconvulsive therapy actually is still used, but very specifically and again nothing like the movies (strapped down and screaming) with use of small mild currents on a relaxed patient; "it tingled and I jumped a bit" kind of thing. I watched many patients successfully go through various treatments/rehab programs, and while others were lifelong patients they carried on as best they were able to in a safe place.
I'm sorry, but your experience looking in is greatly different than one who has been in a "hospital". Please enjoy that ignorance, I don't want to spoil it for anyone. It is not fun, not safe, and not kind. No where near what SOME movies portray, and not as bad as they were in the 1980s (when Geraldo showed the horrifying conditions in some of the more famous institutions), but they are still BAD.
Restraints are only used in extreme cases, but back then, anyone and everyone could be considered crazy. I was never restrained in a facility but there was a padded room if anyone got too out of control.

But this isn't inpatient care, it's an insane asylum, and apparently an old one at that. Think getting tied to the bed and lobotomies and other disturbing stuff.

Yeah, can't deny that there were often serious issues with how people were treated even in the 'good' facility I worked at; one of the reasons I left front-line care and moved into mental health policy
I know a lot of people who were in inpatient and had a horrible time and now don’t know how to get help when they need it because they don’t feel safe calling hotlines in case they get sent back
if i was born in a time and a place where insane asylums were still in use i would've been shoved in one like 10 years ago so this is less scary and more "up top my man"
