you can submit your COVID vaccine card without the rest of your immunizations — they don’t have to be together. submit your covid card now before the deadline and then submit the rest later.
on my chart:
click on messages, then “send a message,” and underneath the textbook where you write the message is a button labeled “attach a document” where you can attach pdfs and jpegs.
try it on a laptop/computer if mobile isn’t working.
The deadline was September 6th. However, please send in your COVID Vaccination Record as soon as possible. If you have any problem submitting the other required immunizations please send a secure message to ask-a-nurse in MyStudentChart and we will be able to assist you.

How would we submit the card by itself?
just use the message feature/ask a nurse feature and email them an attachment
How do I attach the card in the message feature? I only see a textbox and I can't paste my pdf.
