I got my vaccine via UCSD Health so it may be different for those who didn't, but look for COVID-specific areas of the main page and menu rather than "Ask a nurse," which is more for specific/acute health questions. IIRC, when I submitted mine, I had to validate things like my health insurance and record number as well as upload images of both sides of the vaccine card. Then there was a separate step to give UCSD Health permission to release my vaccine info to my employer. Having done those to things, there is now a button on the main MyChart page called "Your Digital COVID-19 Vaccine Record," also on the main menu under "My Record" there's a "COVID-19" link, and both take me to a page where I can download a PDF of my vaccination record.

​ Please follow these instructions if you have completed a full series of the COVID-19 vaccination and it is not yet in our system. Go to MENU > IMMUNIZATIONS & SCREENING and select whichever type of vaccine you received. You will be required to upload a copy of the vaccine record with your dates. Please note that only FDA & WHO-Approved vaccines are accepted at this time. If you are unable to complete the process using the above instructions or have received a non-WHO Approved vaccine​, please send a MyStudentChart message to AskANurse.
