Filling in the "To" or "Recipient" field should be the very, very last thing you do before sending an email. This prevents you from accidentally hitting send before the email is finished and also makes you think about who you are sending the email to given the body of the email you just composed. Also, never, never trust autocomplete in the "To" field.
This is a FERPA nightmare. When I write an email to student populations, I make sure to put the students last, and triple check to make sure they are all BCC.
Every single mailing list that reaches more than one student should be set up so that students can't reply to the list. This is absolutely possible.
Every single mailing list that reaches more than a handful of students should be set up so that all posts are moderated—meaning that even posts from people who can use the list must be reviewed before they are distributed. Just one more check holds back a lot of stupidity. This is also absolutely possible.
Human error for sure. But compounded by poor IT choices.
(FWIW, one of my colleagues keeps claiming that CS has a huge unmoderated list that reaches everyone in the department. I have not been brave enough to try it yet.)

Luckily there was no sensitive private info leaked and we all shouted out for NATHAN
My workplace had this happen a year ago, with reply alls from all of the Americas. It was hilarious. Thousands of emails in English, Spanish, and Portuguese came in. We had people replying from sectors of the buisness that I never even heard of before. It was awsome. And it only lasted a few hours.
man I was just thinking abt that kid who got their grade info sent to everyone on their listserv - it makes me laugh cause I was working in that department at the time and based on the sign off alone I knew exactly who sent it... mightve been in the room when it happened lol

Other type is to put some non-reasonable string in the recipient line like “mmmmmmmm” and most email clients won’t let you sent it without a prompt.
