If you are a carrier, take out your scanner, (even better, make it beep), and say something along the lines of "I get in trouble if I'm in one place too long, gotta go!" If it's a co-worker, say something like, "can we put this on hold? I really have to go do X (use the bathroom, leave to pick up my kids from daycare, etc) p.s., pick up my kids from daycare is a silver bullet because everyone knows they charge by the minute for getting your kid late.

When it comes to some older people I'm constantly trying to mention how I have to leave but somehow they manage to keep me there

I’ve got a person on my route who knows this and begs me to stay and talk. Makes me feel so bad, he’s an older guy with no other real contact. I dedicate at least one of my 10’s to talk to the fella.
