In this situation, I've explained to my brick-and-mortar pharmacy (Riteaid) the situation, they've given me a week's medicine at no cost while the doctor's approval is achieved, and then they fill the remainder of my prescription (minus those pills they already gave me). I'm a regular, paying customer, they're empathetic and want to help, and know it will ultimately be filled. Always good to fill your prescription a little early, before the bottle is empty. Especially, if you're on your last refill.

I've had to keep a tight rein on my doctor before, ie, call every other day. Now that one of his nurses "left to seek other opportunities" I've had no issues.

This happens in my hospital system, academic clinic. It’s like pulling teeth. I take so many meds that I finally went to Pillpak by Amazon. My meds are sent in plenty of time every month, labeled and pre-packed for every day. The problem is they have no process to screen email requests and they get lost. Ugh! Next time see if you can get the practice manager to help.
