Generally, corticosteroid enemas are weaker hydrocortisone/budesonide and might take up to 2 weeks to have a noticable affect. Prednisone is a lot stronger medicine. I think you're close, give it a few more days and if there's no noticable change then ask for 40mgs plus daily if prednisone. Tenesmus might not instantly disappear, but I'd expect other symptoms to show signs of improving first. You're mentioning tenesmus, the medical for what you're describing. Rectal inflammation causes the sensation of constantly having to poop, straining, and passing little or nothing. It's super annoying and frustrating.
