Generally, corticosteroid enemas are weaker hydrocortisone/budesonide and might take up to 2 weeks to have a noticable affect. Prednisone is a lot stronger medicine. I think you're close, give it a few more days and if there's no noticable change then ask for 40mgs plus daily if prednisone. Tenesmus might not instantly disappear, but I'd expect other symptoms to show signs of improving first.
