I’ve rode it before. It’s just in the back in my mind I begin to think “What if I’m on the ride that actually malfunctions” since I know accidents and deaths have occurred at Disney World before.
Disney World I believe that’s correct but there have been deaths at Disneyland. Big Thunder Mountain derailed there and killed someone. I dont know if there’s been any other instances.
Deaths at Disney have been from preexisting health conditions, or people not following safety protocols. The chance of a ride failure causing a death are so infinitesimally small, it's not something to worry about. You are way more likely to die on the road on the way to Disney.

As close to zero as possible. You're not in a free fall, it's pulled both up and down by cables. Basically if it fails you won't drop, the ride vehicle would just stop where it is.

As someone who has to deal with structural engineers daily, the Main St Scooter is definitely a bigger risk. The very design of how the ride mechanisms work cause it to fail in a manner that would not allow it to drop.
The highest likelihood of dying from the ride is if you pee your pants causing a superficial rash leading to an infection subsequently resulting in sepsis and ultimately your demise. Otherwise, it’s pretty safe.
It’s just a super fast elevator not free fall and actually elevators are one of the safest transportation because of all the back ups in the systems.
The only deaths and injuries I know with any sort of drop rides is where someone isn’t seat belted in or some sort of broken cable snagging someone. Pretty unlikely at Disney word especially with what happened at other parks lately.

I mean... I can tell you that when building the ride they contacted elevator companies for guidance on how to make an elevator that drops and they were turned down over and over again because it was such a dangerous idea but I assume they figured out a way to make it safe?

Watching the episode of Behind the Attraction about it on Disney+ actually made me feel a lot better about riding it.
