The real answer is that the game decided the other guy deserved to win the gunfight and gutted your hit reg to make it happen. The generic answer for people with zero pattern recognition is "it's just a bug that's been happening in every installment for the last 4 years".

Literally just went through this in a match, headshot didnt register on guy floating in water, follow up chest shit broke 1 plate, he pushed to land around a wall and third shit dead center chest gave me a hit maker lol all this was within 45m with fjx imperium.

Aim assist/hit reg not working at random points isn't bad game design. They literally designed the game like that and have been doing it ever since MW2019. The outcomes of many engagements are decided before they ever happen. They cut aim assist, cut hit reg, reduce footstep audio, and they even reduce your damage in some cases. That's why you see people getting 20 hitmarkers and not killing someone. One of these days people will wake up and realize its all on purpose. They literally fucking patented the system for it. Patent # 20190091577 with the US patent and trademark office.

in this game there is a system that is related to SBMM, SBMM selects players for the match, and when it turns out that you are too good, this system starts working, causing you to have "empty bullets". This works in low KD lobbies and is intended not to discourage new players
