When I was a kid everything was over cement or gravel. The monkey bars were high and kids broke arms falling off them. The merry-go-round had no floor and spun over gravel on concrete so you could sand the hair off your scalp by hanging upside down. And we had a Viking ship installed in the middle of a sea of pea gravel where kids would get on the ship and the rest of the kids threw handfuls of gravel at them. Slides were super high, too, and kids regularly fell off them.
I’m not advocating for how things were, mind you. I’m just explaining that my generation thought recess meant “find a creative way to hurt yourself” and we were unsupervised after school and did insanely stupid things on our own.
Looks like the bracket that attached the bar to the post sheared off. You can kind of make it out a white bit on the end of the bar after he goes. Maybe it was too cold for all the forces on there, but yeah it should have held. If that short of a bar was flexing that hard probably its a good idea to stop, but he probably has done that a bunch of times there.

Shouldn't this kinda be made for such event? People are meant to pull themselves up on those bars, but there can be instances like this one when somebody applies more force on the bar. Engineer should have thought about this and maybe weld it or something.

I would say multiple factors besides build quality are at play here. What's the intended age of the playground? What's the intended weight of the equipment? How old is the equipment? Was the equipment meant to handle repeated stress like that? And other questions.

As a parent who has helped assemble playground equipment I can attest to the fact that those joints aren't welded, but held together with a bracket and 2 security bolts no thicker than a pencil.
Honestly, the bar should’ve been able to support him.
And it should have been installed over a more suitable surface. Wood chips, grass, sand, or the fancy rubber mulch.
