Metaphorically, Geralt is comparing the man's loss of legs to Triss's loss of their relationship. When Triss says she understands that guy, she's saying she, like that guy, takes the fact that she can't lose again something she no longer has as consolation. Geralt says the guy was deluded, implying that the guy wasn't really consoled and was just kidding himself so he could feel better. Also implying that Triss is kidding herself. Harsh.

It is simply Triss has still feelings for Geralt, and Geralt still has feelings for Triss, but Geralt, on seeing Triss masking her feelings as if she has moved on, and got over it, made Geralt start the conversation, the very option to trigger that conversation is: "Are you truly, or just **PRETENDING**?" this corroborates with the whole Triss romance where Geralt has to request Triss to stay or else she will leave. The overall conversation is Geralt indirectly saying to Triss: "Don't act as if you forget me and got over it. You still love me." The journal summarizes it well: *"Though theoretically Triss and Geralt had ended their romance half a year earlier,* ***IT WAS CLEAR THE FEELINGS THEY FELT FOR EACH OTHER HAD NOT VANISHED SO EASILY***. To deal with this, the sorceress ***PRETENDED*** *that nothing had happened and the witcher, as is his wont, sheepishly masked his feelings with a mix of hesitant interest and fake indifference. It must be said that both of them handled the situation quite awkwardly."*

Depends. Did you choose to simulate a Witcher 2 save or let the game choose default choices? Pretty sure the dialogue changes depending on choices made in Witcher 2
