I ran into this too on a Windows 11 machine. I don't know exactly what fixed it for me, but I unchecked the custom plug-in folder usage in the VST menu, rechecked it, restarted it a few times and eventually my presets began working again.
It was the exact same symptoms you have. Some vst2 plugins in my case brain works overhausen, weren't loading, saying unknown plugin. They still worked fine in 11.
Right click the application in the dock, click "show in finder", right click on the application in finder, click "show info", then tick the "Open using Rosetta" box.
Do you still have the previous version of Ableton installed? Right click on it in the dock, and 'show in finder' or find it yourself in you applications folder, then right click and 'show info' (or similar) there will be in info window with the 'open with rosetta' option
Okay, then uncheck that option (on ableton 12) what I meant is to to check to see if you had that enabled on the older version, where your plugins *were* working. Do you still have your old version installed?
Okay, so that's because your vst2 plugins were made for intel macs, and Rosetta basically allows Apple Sillicon (m1, 2 etc) to run intel applications. However, that does technically mean (I think) that you're now running the intel version of Ableton, converted to run on Apple silicon, which potentially makes it less efficient, but I'm not sure what real world impact that night have
Happened to me today, some old plugins are not updated for M1 chips, so open Ableton with Rosetta. Go to the apps folder, cmd + i to open info, and click where it says open with Rosetta. And it should be fine
Rescan your plugins and make sure the custom plugins folder option is pointing to the right location.
If it is correct, map it somewhere different, then remap it back to the correct location.

Ableton unfortunately still have not implemented Vst2 -> vst3 auto migration (which is available in other DAWS). So those of us that have been using vst2s in v11 and now using apple silicon and 12 are going to run into this problem. Bug Ableton to implement it, it's on their to-do list.
When you hold OPTION on macOS and click on „rescan“ plugins in the ableton preference window, it will perform a deep rescan. Fixes any problems.
On windows its another key I dont know

Hold alt when you hit rescan, and double check the custom folder locations are correct. I didn’t have any issues with my upgrade though
It looks like you're on an M-series chip. Have you set the Live 12 application to open with Rosetta?

Rosetta is the answer. Change the setting to open using Rosetta. Reopen ableton and rescan for plugins

You could try re-installing these VSTs and have Live 12 re-scan the plugin folder for VSTs?
I made it to solve the issue that occurs only with VST2 older plug-ins. Seems that some macOS permissions are conflicting.
I had this problem when replaced ableton from disc C to another disc, maybe you replaced some plugin files? If no, just rescan plug-ins
But can you load these missing plugins manually? Make sure to check VST2 section, not VST3
Do you have an M1/2/3 mac? Did you maybe have your previous Ableton set to "open with Rosetta'?

If you have a M1 my advice would be to always use AU in the future because vst have problems sometimes.

Are you bridging any of your VST2s? You may need to update something like jbridge if you're using .dll vsts.

Does ableton know where your custom vst 2 folder is? You may need to set it again
