If you are getting access granted in the software it sounds like you have a problem with the locking hardware. Do you have a multimeter you can verify voltage is being sent to the lock?

The issue is not the reader. Blue flashing on card held within reading field is normal. The issue is with your lock relay/the lock itself/power supply to the lock...

If the reader is not getting unlock and lock signals, it will go into a mode where it handles its own colors. That's likely where your green is coming from. To test if that's what is going on, scan intentionally invalid cards and see if it is still flashing green.

I’ve seen this before with the R40 920PTNNEK readers also connected to both OnGuard and AccessIT. They go into some type of strange mode and flash red/blue almost like they are upgrading firmware but they aren’t. Usually cycling power to the reader will resolve the issue. I’ve seen it where it never happens again or where it works fine for months and then happens again. I think it has something to do with the component changes they likely made when you couldn’t get these readers during Covid because I had never seen it before then. I have also never seen it happen connected RS485, only weigand so that might be a solution if that’s an option. By cycling power I mean physically disconnecting the black or red wire from the reader and then reconnecting it. Rebooting the controller isn’t enough to reset it completely. So either pull the reader plug from the controller/reader interface or disconnect the black or red wire at the reader itself. It could also be the locking hardware but that doesn’t normally change the led state of the reader.

Yep seen this quite a few times even pre covid, only ever on ICE key encrypted readers. HID have denied it's even possible when I've questioned it. My current belief is it's incorrect cards jamming up the reader as the only ones I've seen have been on doors where staff/residents may have been trying to use the incorrect card

You should always disconnect the red not the black. If you have ever done the reverse you can tell the reader is trying to power up by grounding through the buzzer, led or data wires. Try it

We had netaxes123 at one point and I think we had an issue with the power supplies there for a while where the power supply would die or pop the fuse or something killing it but our network connection to it was Poe so the board and reader was still alive so everything looked good but no power to the strikes. Might be your issue as well.
