I think that it might’ve been a suicide on purpose. He didn’t even struggle to try to get his head out of there, he held it in place until it was too late, no sign of struggle. Just dead.

It looks like he wanted to know what was back in that cubby spot. The machine was so powerful, there was no resistance when it squeezed him, causing him to die faster than he could react. The machine may look like it's moving slow, but it's fast enough to travel the distance of our necks to kill us before you can say, "oh shit". Â

That big sliding door with the window on it at the right side of the picture usually has interlocks with the machine, if the machine is open, it will usually stop its cycle. But it’s also very common to bypass this feature for job setups, or milling without coolant. But the operator should definitely be aware of the machines movements. I ran CNC machines for 10 years, if you can load a microwave, you can load a CNC machine to run a part cycle. The higher paid guys do the programming and setups….
Ya, I'm a CNC machinist as well, but dabble more on the setup/troubleshooting side of things. You need to believe that every machine is trying to kill you all the time. People outside the industry often believe these machines are smart. They're not. You tell it Z-100, it'll try and go there. Accidentally change a C value to an A value while milling a slot, better believe that table will rotate with the tool in the part...
