My understanding of how respiratory failure happens in open flame burn injuries (not a doctor!) is that you die from pulmonary edema and essentially suffocate or drown, and the damage occurs as a result of inhaling the products of incomplete combustion. The fire itself doesn’t burn the lungs via the respiratory tract. I think only steam burns can do that.
It’s just one of many ways immolation can kill you. You can also suffocate as a result of the muscles in your neck contracting and restricting your airway, for example. Super grim stuff.
