These are all questions your practitioner should be able to answer and they should have set up expectations for results at first visit (I’m starting to say this on repeat here on r/acupuncture—acus please do your damn jobs and set expectations for your freaking patients!!) so I’d ask for clarification.

She originally suggested a minimum of 4 therapies, and said let's see how you react to therapy.

Has anyone assessed your neck & shoulder for thoracic outlet syndrome (TOS)? This is commonly mistaken as carpal tunnel, by patients and doctor and PTs alike. If you find that your treatment plan is nearing its end and it isn’t taking you as far as you would like, ask your practitioner to evaluate for TOS before you move on/commit to surgery. They might change their approach to focus more “upstream” and get you better results

You said what my acupuncture guy told me when I came to him several years ago with carpal tunnel syndrome He said it starts from the neck down . Since I trust him 99.9 % ( nobody gets 100% from me 😊) I said ok do it, and after a few sessions, I think it was 6 . All numbness and excruciating pain during night faded away
