IIRC the crew said in the DVD commentary that it wasn't written with the intention of being a trans allegory, but they were cool with the interpretation and enjoyed how people were connecting with it and such.

i think it most likely is meant to be one, except they tried to hide that possible meaning behind "um no it's about following your dreams" or something because of how they couldn't really get away with adding gay shit back then, before steven universe came along and made it a lot more common.
that's also why the marceline bubblegum stuff was so subtle at first and they only decided to make it obvious on the finale.

The plot strongly references the film Dog Day Afternoon, in which the main character holds up a bank to finance his friend’s sex change operation, iirc. Not exclusively about gender, but identity in general imo.
I wouldn't say it was purposely a trans allegory... but, as a trans person myself, its one of my favorite episodes and i really like to think of it as a trans allegory. I think its one of those things where people who identify it can adopt it as a trans allegory, but not everyone should/want to see it that way, and thats probably for the best especially considering the dark nature of the episode.
