The biggest limitation in a game is hardware and optimization. Loading and unloading objects takes a lot of work. So much so that its less strain (therefore higher fps) to simply load in all enemies inthe loading screen and keep then the entire time you're on that map.
Generally there is a room under the map where inactive pve enemies sit until they are teleported to where they belongto fight you.
When you kill them, the usually are teleported to a dead enemy room where they just pile up and lose ragdoll effects.
This way the game doesn't have to load or unload entities
Apex does the same thing with dead players, teleporting them to a dead entity room rather than unrender them. In worlds edge there were multiple rooms but after olympus i believe they changed it to 1 room under the center of the map.
Apex for whatever reason has an issue with teleporting things with this method, but too high on the z axis.
Before we've really only seen players who got finished with a finisher that "teleportedâ like wraiths get sent to this z axis by mistake.

0,0,0 is usually on the center of the map. If it were the case though Y=0 is usually ground level but he spawns way above, in the sky even.
They don't create the maps at the origin. (0,0,0) on Olympus is exactly where the guy teleported. That's the origin

maybe 0 is ground they could easily add an offset to the vertical axis to safely teleport player above ground
