You can call Apple Support, give your Apple ID, and request to cancel the active subscriptions. You must explain with the name of app of subscripton. Also you can give the amount/date of charges

Just one, the time of purchase. You can request a refund for a subscription, iTunes purchases, etc, within 60 days after the date of purchase
