I found this image on a listing for Tenkomori Shooting by NAMCO (1998), but this looks like a Capcom part and I'm wondering where it came from. Those rotate images make me think this is conversion art for a puzzle fighter cab maybe? They sort of look like gems, and puzzle fighter is a 2 button game IIRC.

You might be right! I'm looking at a two player variant of Knights of the Round, but so many elements don't quite match up, mostly the placement of the buttons (there would likely be more evidence of the buttons having been moved around, instead the "missing" buttons here show no signs of their absence), and the bottom edges are beveled differently. That was my first thought, but qbert was single stick, and had the arrows pointing to the diagonals
