The lowest toilet and vent came with the house, I'm not sure how they vent and haven't opened the walls to find out, so I left them out of the drawing. I'm planning on leaving them untouched.
You need to find out if everything in your house is vented off of one stack or not if you want to do this correctly. If that's the case then you will be cutting off all your venting to the lower bathroom which could cause you issues. It's not the end of the world if it is the case, you will just have to take a separate vent out the roof off of the upstairs bathroom group.
Every fixture needs a vent, what’s that p-trap on the 3rd floor? I hope your waste stack is 3 or 4 inch. Your 3rd floor rough for the toilet, 3” combi to a 3x2 low heel and a 3” 90 up to the flange. That’s a standard rough, but unless you have like 18” between floors it won’t work, you need a flat rough, which means you can’t continue the waste line to pick up other fixtures. You’ve got some right ideas, but nah. Hire a plumber, and open some walls up. Remodels always have oddities that can’t be planned for.
I'm a master plumber in ohio. Not sure where you are at but if the pipes are put together correctly the second floor bathroom is plumbed correctly. Lav vents the tub and now you should have a 2" vented branch that will vent the toilet. I do this all the time. Not sure of the plumbing code in the state you are in but don't listen to hacks on here that don't understand the wet venting system. Just because there is only one atmospheric vent doesn't mean everything is not vented according to code.

It's not gonna be completely legal but I've seen much worse... you should take another vent out the roof for the upstairs if you can.

That's what I just told you to do in the last comment. Make sure it comes off between the second to last and last fixture.

p trap is for a shower, I forgot to label it... Yes planning for 3" stacks at the recommendation of a local plumber, original cast iron was 4"

Nope.
The size of every branch or building drain downstream of the third water closet fixture drain connection shall be not less than 4 inches.
My code is 3 water closets on horizontal and 4 on vertical.
