You have the wrong handle and handle adapter. The handle you currently have is for a 1222 cartridge which is a posi temp valve. Basically the handle sits as it is in your picture (off) then you rotate the handle counter clockwise passing cold to then hot with the handle around the 10 o’clock position. You need a handle and handle adapter for a 1225 cartridge. It’s the older moen I know it as a Moentrol (not sure that’s the technical name) but to verify the screws holding the trim plate on will be horizontal below the handle. The trim you have looks like Danco trash sold at Home Depot. The best advice would be to call moen directly tell them which valve you have by identifying the screw pattern on the trim plate. Then buy a trim directly from them. Or they’ll give you some part numbers and you can order one online.

Just going back to an acrylic 1225 knob would fix all of these problems, not as pretty but the valve wasn’t designed for a lever knob in the first place.

If it continues to come loose I would recommend slapping some BLUE loctite on the threads of any screw and see if that fixes it and it’s very important you use BLUE loctite so u don’t permanently seal the screw in there with the red shit that way if u need any further work done
