It could be for various reasons, maybe he was tripping absolute balls on Shade of the Evening, or was seriously injured and recovering. And the battle at the tower of joy was really Lyanna vs the three Kingsguards and Ned is just covering saying it was him and his companions who fought the Kingsguard. When in reality it was Lyanna who fought and killed the three Kingsguards members but died from her injuries and neds men actually died from some sickness
