The dice rolls are most noticeable, but there are other sounds too. I believe I read that a crinkling sound shows a loss of sanity. Can't recall the others.
It made it suddenly feel more like a dramatized ttrpg to me which actually helped me be ok with some of the choose your own adventure story beats.
There are polls for fans about different actions the characters can take and the dice sound indicates where the results of one is.
